Our vision is of a health and care system that has excellent care at the heart of the community.

We are the specialist provider of community health and care services to the people of West Sussex and Brighton & Hove. Every day our expert teams provide medical, nursing and therapeutic care to more than 8,000 people.

Across the age range, we care for some of the most vulnerable people in our communities, from our health visitors caring for babies and young children through to our multi-disciplinary community teams caring for the frail elderly and people at the end of their lives.

In all we do – and in our ambitions to achieve the very best for the people we serve – our work is driven by our vision of excellent care at the heart of the community.

To achieve our vision we want to raise awareness of the value and range of what we do and develop a reputation for excellence and innovation so that no one can doubt the central role that community healthcare services play in the local NHS.

We are taking forward our ambitions for our public and patients through a transformation strategy which focuses on quality improvement and assurance to deliver increased productivity and cost efficiencies.

This strategy – driven by our vision and values – with our quality and clinical care strategies at its core, is informed by national and local strategic drivers and supported by a comprehensive range of clinical and corporate strategies.

The aim is to deliver the right services to the right people, in the right place, by the right people and at the right time, quality and cost’ by ensuring that the organisation has in place the right strategies, structures, systems, staff and skills, underpinned by shared values.

Our vision of excellent care at the heart of the community points to where we want to get to, and what we want to be. To move us in this direction, the trust board has set three strategic goals. These explain what we need to do to achieve our vision:

  • We will provide excellent care every time to reinforce wellbeing and independence.

  • Working with our partners we will personalise services for the individual.

  • We will be a strong sustainable business, grounded in our communities and led by excellent staff.

One of the key ways we will work to achieve our objectives is to become an NHS Foundation Trust (FT), and we aim to achieve FT status by 2015.

Clinical quality is central to the care we deliver. Our clinical care strategy explains how we intend to make our services easy to use, safe, consistent, effective and right for each individual. The core of our strategy is to provide excellent clinical care, delivered by experienced and trained staff.
